Annual Volunteer Registration form

Dear Parents/Guardians, In addition to the Volunteer Police Security Clearance from CBE, we also require an Annual Volunteer Registration Form to be completed each year by anyone volunteering within the school.  For those who completed a police clearance last year or at a previous school, you will not need to redo the police clearance portion, and we DO NOT need to see your identification again.  Simply complete the form and return it to your student’s teacher if you plan to volunteer again this year. If you are interested in becoming a volunteer, please complete the form and return it to the school office IN PERSON as we will also need to take a copy of one piece of identification (Driver’s License, Passport) to process your police clearance letter.
